Due to our public office closures, we are accepting hearing requests in writing by fax, email or mail. The following information should be included to reduce delay in processing the request:
- Full name
- Date of birth
- License number
- Correct mailing address
- Whether you would like the officer present at your hearing
- A copy of the Notice of Revocation/Suspension, if available.
- This will speed the processing of your request. This can be a scanned document or a photo of the notice. Please be sure the entire document is legible in the photo.
- For Administrative Insurance Notice of Suspension, proof of current insurance must also be provided along with the request.

A confirmation of receipt automatically is sent after an email is received. Once the request is processed (no matter the delivery method), a letter is sent via USPS to the address provided in the request. Processing typically takes about 1-2 business days from date of receipt.
Hearing requests related to anything other than a new DUI, Administrative Insurance Notice of Suspension, or Probable Cause for Hit and Run Collision and Order of Suspension should be directed to the Hearings Division at firstname.lastname@example.org or 303-205-5606.
