Driver Services (Driving Records, Suspension, Revocation, Cancellation, Denial, OJW, Alcohol Hearing, SR-22, Probationary (Red License) and Interlock)
Identifying license plates or placards can be issued by submitting a completed Persons with Disabilities Parking Privileges Application (Please find Form DR 2219) to your County Motor Vehicle Office. Placards can also be issued or replaced through our online system.
Driver License Offices are not able to issue disability parking placards or license plates. If you need additional assistance with your Disabilities Parking Privileges Application (Please find Form DR 2219), please make an appointment with your County Motor Vehicle Office.
