If your driver license or identification card number has been compromised, a new personal identification number (PIN) may help you conduct business. However, because motor vehicle records are protected by federal and state privacy laws, they are not accessible for credit purposes and will not prevent or stop credit fraud. To obtain a new PIN, please do all of the following:
- Download and complete the Affidavit of Colorado Driver License or ID Theft form (Form DR 2153)
- Have your signature notarized
- Take (do not mail or fax) the Affidavit form with your police report to any Colorado driver license office
