Replace Your Lost, Stolen, or Destroyed Driver License/Permit/CDL/ID
Situations can arise that might require you to get a new driver license, permit, CDL, or ID outside of the normal renewal timeline.
Replacing a Driver License/Permit/CDL
You may request a duplicate driver license if it is lost, stolen, or severely damaged at a lower fee. A duplicate is a copy of your original license or permit, with the same information and expiration date. For more information, click here.
Replacing an Identification Card
If your ID is lost, stolen, or severely damaged, you will need to renew your ID. Depending on your situation, you may need to take additional action. For more information, click here.
Replace a Credential Sent by Mail
If the credential you were expecting to receive has not been received within 30 days, you can ask for a replacement at no cost. You must request the replacement within 30-90 days from issuance.
Check the status of your mailed credential online via myDMV's "Where is my Driver License/ID."

If you believe your credential is being use to conduct fraudulent activity, such as identity theft, you should contact law enforcement before contacting the DMV. For more information, click here.
Important Information
You can get a duplicate credential if your credential is lost/stolen/destroyed. Duplicate driver’s license/permits/CDLS are not available online.
In Office
- Make an Appointment Online to Visit a Driver’s License Office: Schedule your office visit by clicking here: Appointment Scheduling. Select "Renew Colorado License/ID/Permit” as your appointment type.
- Attest to Loss/Theft: During your visit, you'll need to confirm that your credential was lost, stolen, or destroyed.
- Verify Your Address Information: When receiving a duplicate, you will be asked to verify your address is correct with the technician.
- Bring Proof of Identity: You will need an identity document, which can be found on the Required Documents page. If you do not have proof documents you will be asked to provide the last 4 digits of your Social Security number and we will attempt to match your fingerprint.
- Fee: There's a fee for a duplicate license. Information about fees can be found here.
By Mail
A duplicate by mail may be requested by holders of a Real ID compliant credential who are temporarily out-of-state and provide an out-of-state mailing address (Exception: a duplicate of a Real ID compliant Commercial Driver License issued to a Temporarily Lawfully Present applicant must be requested in person. This is not available to Temporarily Present or Non-Lawfully Present individuals.)
To request a duplicate by mail:
- Complete the Form: Fill out DR Form 2989 from our Forms page. The form should include your out of state address in order to be processed.
- Mail the Form and Fee (Check or Money Order) to:

Early Renewal
If you want to skip the trip to the office, you may consider renewing your driver license early online (if qualified). Doing this will extend the expiration date of your credential. Check your eligibility to renew online at myDMV. You will not be eligible to renew online if your license has expired for over one year or if your picture is 10 years or older.

If your ID has been lost, stolen, or destroyed, your credential will be processed as a renewal. If your picture is less than 10 years old, you may be able to renew online. If your picture is more than 10 years old, you will need to make an appointment.

If you do not receive your physical credential within 30 days from issuance you can:
- Track your credential
- Email the returned credential office at dor_dmv_dl_returned_credentials@state.co.us
- Call 303-305-5600 (press 1 then press 3)
- Return to the office that issued your credential
Note: If not claimed, the DMV destroys un-deliverable credentials after 90 days from the date of issuance. Customers will then need to purchase a new credential at full price.
How to obtain a replacement:
- You may be eligible for a replacement credential at no charge if you did not receive your original credential in the mail.
- To get a free “lost in the mail” replacement, please visit a state driver license office between 30 and 90 days from the date the license or permit was issued.
- An appointment is not needed if you have not received your credential in the mail after your temporary paper credential has expired, and as long as you are within the 90 days of the temporary’s issuance.
- If 90 days have passed, and you have not received your credential, and you have not come into the office for a replacement, you will have to pay for a duplicate credential.
What you need:
- Verify Your Address Information: When receiving a duplicate, you will be asked to verify your address is correct with the technician. If your address needs to be changed, you must show us proof of your new address. Proof of address requirements can be found on the Required Documents page.
- Bring Proof of Identity: You will need an identity document, which can be found on the Required Documents page. If this cannot be done, you will be asked to prove your identity. You will be asked to provide the last 4 digits of your Social Security number and we will attempt to match your fingerprint.
- Fees: Fees vary depending on the type of card (DL, ID, CDL) and the renewal period. Find more information on the State DMV Fees page.
Important Information:
- We can not duplicate credentials documents from outside states.
- Your missing credential will be invalidated.
The Division of Motor Vehicles does not place alerts on motor vehicle records for lost or stolen identification cards or driver's licenses. However, there are security measures you can take to protect your personal identifying information detailed here: Lost, Stolen or Compromised Documents. An example of a misused or compromised license or ID card would be if an unauthorized person used your credential without your permission and opened a bank account or line of credit that you didn’t authorize.
If you have reason to believe your identification card or driver license was stolen, you should report that to your local law enforcement agency.
If you have reason to believe your identification card or driver's license number is being misused, you can request a new identification/driver's license number. Here's what you need to do:
- Complete the Affidavit: Download and complete the Affidavit of Colorado Driver License or Identity Theft (DR 2153 Form) on the Forms in Number Order page.
- Get Notarized: Have your signature on the affidavit notarized.
- File a Police Report: Report the misuse to your local law enforcement agency. You must bring an itemized police report to the Drivers License office in addition to the Affidavit. We do not accept case numbers, you have to have a counter or full police report.
- Make an Appointment and Bring Documents in Person: Make an appointment (select the Renewal appointment type) and bring these documents to any Colorado Driver License office. Do not mail, email, or fax them.
- Proof of Identity: You will need proof of identity like a prior license or passport at the driver’s license office.
Note: This process is irreversible once the new credential number is issued, the lost drivers license/ID becomes invalidated.
